Short-Term Rehab

Physical, Occupational, and Speech Therapy

Diversicare facilities have teams consisting of physical therapists (PTs), occupational therapists (OTs), and speech language pathologists (SLPs) who work together to provide focused, individualized treatment and rehabilitation for our patients.

• Physical therapy addresses posture, balance, strength, and walking.

• Occupational therapy focuses on daily activities such as bathing, eating, dressing and toileting as well as instrumental activities of daily living such as home management and community reintegration.

• Speech therapy addresses swallowing problems, memory disorders, and communication challenges.

All treatments are provided to regain the highest levels of function. The patient and their family members are actively involved in all aspects of the rehabilitation process, from goal setting to discharge planning.

Stroke and Neurological Disorders

Neurological dysfunction is caused by damage to the nervous system and may affect a range of body systems. Such as the brain, spinal cord, nerve roots, junctions between the spinal cord and the muscles, and extremities. These lesions usually occur as a result of trauma or disease.

Orthopaedic Conditions

Orthopaedic conditions involve the bones or joints and may include fractures, muscle tears or lacerations, joint repair or replacement, and amputations. The majority of fractures in the elderly are the result of falls and the effects of osteoarthritis.

Comprehensive Wound Care

Skin breakdown can occur quickly with the elderly due to tissue fragility. Our comprehensive wound care program begins with preventive measures to avoid skin abrasions, pressure areas, and shearing. If a skin condition should occur, our highly trained nursing, dietary, and physical therapy professionals work together with the resident’s physician to ensure that optimal healing measures and treatment are implemented.

Rehabilitative Feeding Program

The ability to maintain adequate nutrition and hydration is essential to sustaining life. However, this daily need is not only physical in nature, but also social and emotional. Our rehabilitation dining program includes two phases, plan development and implementation by occupational and speech therapy professionals and plan maintenance by the restorative nursing staff.

Post-Surgical Recovery

Several factors can affect an individual’s ability to recover following surgery, including age, medical stability, and other pre-existing medical conditions which may complicate recovery. Our skilled nursing and rehabilitation staff individualizes each resident’s plan of care to accommodate specific needs and restore functional independence.

Comprehensive Discharge Planning

Residents and their families are actively involved in the discharge planning process upon admission and throughout treatment. One of our physical or occupational therapists can perform an on-site home evaluation with the resident and their family prior to discharge to identify potential safety issues, equipment needs, and environmental adjustments to ensure a positive transition into the home.

Acute Pain Management

While medication is often the most appropriate pain relief intervention, treatment methods should be matched to the resident’s lifestyle needs and goals based on a thorough pain analysis. Pain management techniques and relaxation strategies, such as biofeedback, massage, and heat or ice application, can offer relief from debilitating pain symptoms. Our nursing and rehab professionals are highly qualified in all aspects and modalities to promote individualized pain management.
